Recording research results and sources

My question is more about recording research than doing it at the moment .
I use blank records forms I found on TS several years ago - Certificates and other sources: Individual summary sheet etc .One has "created by Andew McDonald 2006" on it

But I can't remember/find the link on TS - I had to take a break from my research but am now returning

I found/find these sheets invaluable - many many thanks for them - are they still accessible ?? And if so, where?
