Where have you got to with it so far? You know the child's name, you know the date of birth, the wording follows a reasonably familiar pattern by the looks of things - have a go!!
I'll try to help if you get stuck, but I'm no expert in secretary hand, I just dabble when needs must!

John Bell had a son baptised [not sure might be] callit [as in called] James. Witness?[looks like plural, I think...but then there only seems to be one!?] James Bell portioner? then possible placename, maybe??
